LIABILITY INSURANCE
[O.S. Title 47 § 7-600 to 7-610] [Rule 710:60-3-17]
The owner of a motor vehicle to be registered or titled in this
state must surrender a current Oklahoma insurance verification
form prior to the processing of his registration, unless
otherwise provided.
To be acceptable for vehicle registration purposes, an insurance verification form must contain all of the following information:
1 . Name and address of the carrier.
2. The name and address where security may be verified, if other than carrier.
3. Name of insured.
4. Notice that an owner's policy has been issued pursuant to Compulsory Insurance Law
of Oklahoma.
5. Year, make and at least the last three (3) digits of the VIN of each insured vehicle.Note: If owner's insurance verification form states "Fleet Coverage", year, make and last three (3) digits of VIN are not required.
6. Inclusive dates liability policy is in effect. Failure of the customer to keep current insurance coverage is not the motor license agent's responsibility.
7. A warning to the vehicle owner about the state law requiring one (1) copy of the verification form to be surrendered upon registration and one (1) copy to be carried
in the vehicle. If a taxpayer submits a verification form that does not reflect this
warning, the form is not acceptable.
Note: Section 7-601.1 of Title 47 of the Oklahoma Statutes establishes the requirements for liability insurance security verification forms. The seven (7) items listed above are specifically required by statute. The absence of any of those required items renders a security verification form unacceptable for vehicle registration purposes in Oklahoma. Photocopies ofsecurity verification forms are unacceptable, unless the original is presented to youfor inspection. Facsimiles (FAX) are acceptable only if all seven (7) required items are listed on one sheet. In other words, two FAX sheets, one of the front of the verification form and one of the reverse side of the form, are unacceptable.
Although the name of the owner of a motor vehicle may not match the name on the insurance verification form, the form may be accepted, provided all other information is accurate.
