The following is the proceedure to order a Duplicate Title from your local tag agent online. Fill out the following information and submit it. The Duplicate Title form will be made available to print at that time. Print the form, fill in all information and fax it to your tag agent.

The Duplicate Title will be mailed to you unless other instructions are provided.
