Specially Rated Tags Registration Information

REQUIREMENTS: The following is needed to register:

  1. Either your Tag number, title number or VIN (Vehicle Identification Number). At least ONE of them is needed.
  2. Your insurance information: company name, policy number and the name of the policy holder. You will also need to fax us a copy of your Insurance Verification Card. The number is (918)369-4640.
  3. If your vehicle will not be used on public roads, complete this form except for the insurance portion, then download the Non-Use in Lieu of Liability Insurance Affidavit Click Here. This form will need to be properly filled out, notarized using a visible stamp (not embossed seal), and faxed using one of the above listed fax numbers. (An additional charge of $1.00 will be charged to your credit card.)
  4. If you are an Active Duty Military Personnel either residents of Oklahoma or are stationed in Oklahoma, complete this form, then download the U.S. Armed Forces Affidavit. Click Here. This form will need to be properly filled out, and faxed using one of the above listed fax numbers. (Your Credit card will be charged $25.25).*
  5. If you are Physically Disabled, complete this form. Be sure to include the number from the Handicap Placard in your vehicle. Then download the Affidavit for Physically Disabled. Click Here. This form will need to be properly fill out, notorized using a visible stamp (not embossed seal), and faxed using one of the above listed fax numbers. (Your Credit card will be charged $35.75).*
  6. If you are renewing a Commercial Plate, fax a copy of your Schedule C (both sides) from your most recent Federal Income Tax Return, Federal ID Number, or Sales Tax Permit number using one of the above listed fax numbers.
  7. If you are renewing a Farm Plate, fax a copy of one of the following: Schedule F of your most recent Federal Income Tax return or your SMX number (fuel tax permit number). (Your Credit card will be charged $39.75).*
  8. Credit Card Information. You will need to know the type of card (ie. Visa, MasterCard or Discover), card number, expiration date, name, and billing address as it appears on the card.

* If late fees apply then the above charges will vary.

Once you have all of the above information, you can then renew your tag online.
